DFD – US 250 Badge – Order Form

To use this form, fill out the editable fields then click the download button and select "With your changes" to download the completed form. Once downloaded send the form as an attachment to orders@sunbadgeco.com.

Important Information

• Badges may only be purchased by Retired or Active Denver Fire Members
• Members may purchase badges for current or previously held ranks
• All purchased badges will be sent to DFD Headquarters for inspection before being released to the purchaser
• Badge cost is $100, the price will increase on January 1st, 2027
• Turn around time is approximately 14 weeks from purchase
• Members are authorized to wear badge on duty through July 4th 2027
